Knowledgeable, Experienced, Caring Staff
BridgeWays trained and licensed team includes both Case Managers and Peer Supports who are dedicated to providing excellence in case management and support services. From discussing everyday problems to handling critical situations, BridgeWays staff will listen and take action. During operating hours, there is a Qualified On Call Case Manager to address immediate concerns.
Individual Attention and Planning
BridgeWays assists individuals in designing a team of service providers that will help them define and reach their goals, realize their potential as full participants in the community, and move down their own path toward a self-directed future. BridgeWays Case Management team provides comprehensive assessments and Person Centered Planning to achieve identified goals; medication monitoring; assistance with obtaining housing and maintaining safe housing; assistance with obtaining and maintaining benefits; linking to and from vital appointments necessary for health and independence; help in budgeting and coordinating transportation as well as providing referrals to community resources for social, educational, vocational and recreational growth and development.
Education
The BridgeWays team provides education about diagnoses, medications and their side effects, and other health-related issues to participants. Family members and friends can also receive education about mental illness, support, and referrals when appropriate.
Referrals and Coordination
BridgeWays assures quality, seamless care by assisting, coordinating, and making referrals to a variety of services available in the community, including: medical, dental, employment, housing, transportation, pharmaceutical, financial, psychotherapy, substance abuse, and social activities.

BridgeWays is conveniently located at 1606 South Burdick, on the South Burdick Metro Transit bus line directly across from the Michigan Works building.
